Death Benefit (Urgent)

Urgent news regarding the Death Benefit

With Local 12 moving to Centralized Dues with DC16, your death benefit fund is no longer consolidated into your dues. In the years past you normally made a quarterly payment $123 if you were a Journeyman or apprentice; and $102 if you were a Floor Preparation Worker or Floor Covering Handler. Weather you paid the $102 or the $123 rate, $18 of that was for your death benefit. Going forward, all payments for the Death Benefit Fund need to be made directly to the Local one of two ways;

  1. You may pay with Debt/Credit card by calling the Union Hall: 408-824-1280.
  2. Or; mailing or dropping off a check to the Union hall: 2149 Oakland Rd. San Jose, Ca. 95131       

The amount for the death fund is still the same, $18 for the quarter or $72 for the year. Your death claim cannot be processed if you are not paid up to date, so If you have not made a payment yet this year or if you are unsure if you did, please contact the Union Hall as soon as possible.
