To participate, you must first select a Smart Choices Plan on your enrollment form.

Once you receive your medical ID card, both the member and spouse must complete ONE option below.

Anthem Blue Cross / Indemnity Plan

Option 1: District Council 16 Trust Fund Online Health Assessment (HRQ)

• Call Health Services Foundation: 1-888-243-3216 
• Complete the assessment over the phone 

Option 2: Biometric Screening (Quest Diagnostics) (Form Required)

      In order to complete the bloodwork through Quest Diagnostics

1. Visit: www.My.QuestForHealth.com 
2. Register your account 
3. Download the physician form 
4. Schedule an appointment 
5. Have your doctor complete the form 
6. Submit the completed form to the Trust Fund Office (TFO) 

Kaiser Plan

Option 1: Online Health Assessment or Course

1. Log in at: www.kp.org 
2. Go to Health & Wellness → Tools for you (Programs & Classes) 
3. Select Total Health Assessment → Start Health Assessment 

Note: Kaiser updates eligibility monthly. To speed up processing, submit your Wellness Score with your enrollment form.

Option 2: In-Person Class (Form Required)(form is available on website or Trust Fund Office)

• Attend a Kaiser class 
• Have the instructor complete and stamp your form 
• Submit the form to the Trust Fund Office (TFO)
