INSTRUCTIONS<\/strong><\/p>\n\n\n\n

Elect one of the Smart Choices Plans on the enrollment form first to enroll; once you get a medical id.<\/p>\n\n\n\n

card then you can do one of the screenings. Both member and spouse are required to complete<\/strong><\/p>\n\n\n\n

one of the options below:<\/strong><\/p>\n\n\n\n

Anthem Blue Cross\/Indemnity Plan:<\/strong><\/p>\n\n\n\n

Option District Council 16 Trust Fund Online Health Assessment:<\/strong><\/p>\n\n\n\n

Call Health Services Foundation at 1-888-243-3216 and they can do the HRQ over the phone.<\/p>\n\n\n\n

Online options may be available at a later date.<\/p>\n\n\n\n

Option Biometric Screening \u2013 Quest: <\/strong>(Form Required)<\/em><\/strong><\/p>\n\n\n\n

In order to complete the blood work through Quest Diagnostics, go online to<\/p>\n\n\n\n

www.My.QuestForHealth.com register, *download a physician form <\/em>and make an appointment. Your<\/p>\n\n\n\n

physician will need to complete the form; you will need to submit the form to the TFO.<\/p>\n\n\n\n

Kaiser Plan:<\/strong><\/p>\n\n\n\n

Option Online Health Assessment (HRQ) or Online Health Improvement Course:<\/strong><\/p>\n\n\n\n

Log on to www.kp.org or https:\/\/healthy.kaiserpermanente.org\/northern-california (you must be<\/p>\n\n\n\n

registered) and go to the Health & Wellness tab. Scroll down to Programs & Classes. Click on \u201cTotal<\/p>\n\n\n\n

Health Assessment.\u201d Click \u201ctake a Total Health Assessment\u201d and click on \u201cStart Health Assessment.\u201d<\/p>\n\n\n\n

Please be advised:<\/em><\/strong><\/p>\n\n\n\n

[Kaiser processes eligibility updates through a monthly file. If you would like to expedite the update to<\/em><\/p>\n\n\n\n

your plan, you may submit the \u201cWellness Score\u201d with your enrollment form.]<\/em><\/p>\n\n\n\n

Option in Person Class: <\/strong>(Form Required)<\/em><\/strong><\/p>\n\n\n\n

Use form which includes additional instructions to go into Kaiser and do an in-person class, the<\/p>\n\n\n\n
